Thank you for your interest in the High School West Virginia Mission Trip as we partner with the high school ministry of Grand Ledge Baptist Church and are lead by YouthWorks! As we get closer to the trip, there will be additional forms beyond this registration that will need to be completed during one of our mandatory meetings.

Please complete this registration form and pay your deposit of $200. Once you have registered, you'll receive additional information about required waivers, health forms, pre-trip meetings, a packing list, remaining payment information, and other key deadlines.

As a reminder, if the cost is prohibitive, speak with Annastasha or Greg about scholarship opportunities. We want you on this trip!

Trip Costs

The trip costs are outlined at the top of this application. Trip costs cover transportation, housing, food and activities. Additional expenses you may incur would be any snacks or souvenirs that you want to buy. The total cost of the trip is due prior to departing.

Other Acknowledgements

All information on this application is accurate to the best of my knowledge. As a participant of the Riverview Church (Riv) trip, I hereby release Riv, its agents, employees, and volunteer assistants from liability for any accidents, sickness, ransom or death that may occur while service with Riv. I authorize and give permission to Riv and its agents to take me to any doctor for medical treatment, emergency surgery, and hospitalization or care if needed. I assume responsibility for all medical bills incurred. I take full responsibility for all travel shots or oral medications that are required for this trip. Should it be necessary for me to return home before the end of the trip, I agree to assume total transportation costs. I certify that I am of lawful age and competent to sign this release and have done so voluntarily. I understand this electronic and/or physical document constitutes a full and complete waiver of all claims for any act or omission, including claims for negligence regarding injury, damage or loss which may be sustained by me during the course of my involvement with Riv.
